STATEMENT: I have read and answered all questions truthfully and understand that falsification of information on this application will result in automatic rejection. I permit investigation into my background. I am willing to take an oath of allegiance to the constitution and bylaws. I understand the application fee is not refundable if the application is denied. I further understand that I must be present at both the first and second reading of my application, and if I do not present myself within 6 months of the first reading, the initial fee is forfeited and I must submit a new application if I desire membership in the CFLA. You are not required to furnish requested information, however, the information requested is deemed necessary to conduct a proper investigation of your qualifications to become a member of this association.
